If you miss your Administrative Law Judge (ALJ) hearing, you must submit your COMPLETED HL 90 Form to the judge's office within 10 days (plus 5 days for mailing) from the date of the judge's post-ALJ hearing Notice otherwise the judge may terminate your Disability claim.

If the Social Security Administration (SSA) sends you a notice informing you that your Social Security Disability Insurance (SSDI) and/or Supplemental Security Income (SSI) benefits will be terminated or suspended, you must send the SSA a COMPLETED SSA 795 Form, within 10 days (plus an additional 5 days for mailing), declaring that you desire to maintain your full SSDI and/or SSI benefits while you appeal the decision to terminate or reduce your beneifts.
